APPLICATION OF NURSING THEORY 2 Application of Nursing Theory: Medication Adherence in Chronic Illness As nursing has evolved, so has the theory behind it. Nursing theory uses foundational concepts to explain, describe, and attempt to predict nursing beliefs, goals, and actions (Schaffer, Sandau & Missal, 2017). There are many nursing theories which seek to explain phenomena or enhance practice. Nurses have been applying nursing theory to practice for decades and as a family nurse practitioner (FNP), I will do the same. One issue that theory can be applied to is medication adherence in chronic illness. I believe that Betty Neuman’s systems theory can be applied to this issue. The Neuman systems model is both a grand theory and a conceptual model (Alligood & Tomey, 2013). As a theory it gives nurses a way to view phenomena as well as nursing actions; as a conceptual model it can help guide nursing practice (Alligood & Tomey, 2013). There are several concepts included in this mode, the client is seen as a unique system which is interacting with other systems and reacts to stressors which can be either internal or external in nature; these stressors work on lines of defense and resistance around the basic structure of wellness (Alligood & Tomey, 2013). How a client reacts to these stressors can define the level of illness and the interventions focus on prevention at the primary, secondary and tertiary levels (Alligood & Tomey, 2013). There will be a description of the issue of chronic illness and medication adherence as well as application of Neuman’s systems theory with a self-reflection. Description of Issue An FNP works with patients of all ages, ethnicities and backgrounds. One problem that has presented itself is the issue of medication adherence in patients with chronic illness. In the United States there has been a trend of high prevalence for chronic diseases; in 2012 half of all

APPLICATION OF NURSING THEORY 3 adults had at least one chronic condition and one in four had two or more (Chapel, Ritchey, Zhang & Wang, 2017). These chronic conditions include, but are not limited to: coronary heart myocardial infarction, heart failure, pulmonary heart disease, angina, hypertension, dyslipidemia, diabetes, cancer, asthma, chronic obstructive pulmonary disease, emphysema, mental disorders, obesity, arthritis, renal disease, or any combination of these (Chapel et al., 2017). This becomes important for the FNP to understand as this will account for nearly half of the patient population seen. This population generally requires monitoring, medication and follow-up care for management and prevention of exacerbations of the condition.
