Tracer methodology allows surveyors to determine whether there is consistent provision of appropriate and safe access to care, treatment, and services. (Healthleadersmedia staff, 2009) Tracer methodology is an evaluation method in which surveyors select a patient, resident or client and use that individual’s record as a roadmap to move through an organization to assess and evaluate the organization’s compliance with selected standards and the organization’s systems of providing care and services. Accurate, complete, legible, and timely medical records will continue to be essential elements for periodic performance reviews (Insider, 2006). Documentation must be as timely and complete as possible. The medical record should accurately reflect the patient's care from start to finish. Late or illegible entries and otherwise poor documentation do not contribute to the quality and safety of patient care (Insider, 2006)..
