FORMAL REFERRAL PROCESS 2 Formal Referral Process Introduction The following information depicts the procedures and processes a case manager often considers or should consider using during the completion of their formal referrals of clients to the appropriate community services, which are necessary to aid them in their recidivism efforts. It also emphasizes the extreme importance for these case managers to link their clients to these services before their paroles or probationary periods to ensure that their chances of becoming repeat offenders are lessened. We desire that everyone that read the following information will be enlightened by the vast amount of information necessary for the case manager's successful planning and implementation. The case manager's detailed plans, considerations, and contacts needed for individualized problem solving and treatment of these clients as they move forward. Case Manager Processes and Procedures The systems a case manager must take when completing a formal referral ensures that the client is ready for the next step of their recovery and re-entry to society. The procedures require and allow both the client and the case manager the necessary tools needed to maintain structure, futuristic goals, and accomplishments. The partnership between the case manager and the client will grow stronger through these procedures, as they will learn the effectiveness of the steps to come.
