NURSE LEADER 2 The purpose of consultation is to identify issues and ways to resolve them in a dialysis clinic. The consultation process is a cooperative problem-solving process as an organization. The task as members of the consultation process is to respond to emerging issues. In order to have an effective consultation process you need to have people that will be affected by the issues. By researching the clinic and preparing an interview for the nurse leader a change can be identified and recommended for the clinic based on the findings. After research and an interview, the consultant can propose a resolution to any weaknesses identified. (American Association of Colleges of Nursing, 2011) The dialysis clinic is one clinic in a large company that services inpatient and outpatient renal care throughout the country. This dialysis clinic is located in a large urban city in Central Indiana. This dialysis clinic has 20 chairs, meaning that up to 20 patients can run each shift. That means that this dialysis clinic can do up to 300 dialysis treatments a week, or over 15,000 a year. It is a for profit company that services thousands of dialysis patients a year in over three thousand dialysis clinics. The company employs over one hundred thousand people. They are accredited with the Joint Commission and CMS. (Fresenius Medical Care, 2018) The primary need for a dialysis patient is to have their body cleaned from toxins that have built up from the kidneys not working properly. The typical dialysis patient has had damage to their kidneys due to another illness such as diabetes or hypertension. Once the kidneys are damaged they are not able to be repaired. The kidneys are responsible for filtering the toxins from the body as well as excess fluid. When the patient’s body is not able to do this on its own, toxins and fluid will build up in the body and cause issues for the patient, such as difficulty breathing or heart issues. The typical dialysis service area is everywhere. With 40,000 new patients a day starting dialysis, you now see dialysis clinics in every area, rural, urban, and

NURSE LEADER 3 suburban. Patients are kids as well as adults, the one thing every patient has is common is their failing kidneys. Some patients have damage from high blood pressure, diabetes, chemotherapy treatments, acute injury to the kidneys from an accident, and some are born with damage or diseased kidneys. One of the main risk to the patient on dialysis is other health issues. Many of the patients that pass away while on dialysis do so from some other issue. Not being able to properly clean toxins and excess fluid from the body has a negative effect of many other organs. One of the main issues are respiratory failure or heart failure. (T.Vest, personal communication, February 23, 2018) 32% of the population in Marion County are considered to be obese, and this has gradually increased over the past few years.
