Describe your state’s efforts and progress in the Healthy People 2020 initiative. Identify the coordinators of the efforts. Summarize the leading health indicator programs and report on the progress that has been made. Practice critical thinking skills by sharing your own personal thoughts and insights on the health disparities and leading health indicators you have either observed or are surprised to find exist in your state and community. Since I have been in the In-Home healthcare field for the last nine years I have come across several patients with chronic diseases here in Virginia for instance some of my patients that I have had the pleasure of taken care of these nine years have had these chronic diseases COPD, Strokes, Seizures, Cancer, Heart Disease, Liver Disease, Asthma, Alzheimer’s, Dementia, Diabetes, as well as Arthritis and Rheumatoid Arthritis. The leading cause of deaths in the state of Virginia is Chronic Diseases. Throughout the United State as well as Virginia the treatments for all these Chronic Diseases are extremely high.
