HEALTHCARE BUSINESS AND FINANCIAL MANAGEMENT 2 Principles of Healthcare Business and Financial Management A1 Current Status This authors currently facility has implemented value based purchasing (VBP) through incorporating an electronic health record (EHR) and implementing clinical decision support systems that directly impact the hospitalization rates, reduce the length of stay and provide better outcomes. The EHR has several areas that are directly in-line with the Centers for Medicare and Medicaid Services (CMS) VBP program. These areas focus on the CMS measure and give practitioners build in notifications and hard stops that require attention prior to allowing the practitioner to proceed into a patients chart. These stops address the CMS measures such as removal of Foley catheter one day post op or administration of beta-blockers given to post- operative patients who receive a beta-blocker the day before surgery. These areas will create a pop up window that draws attention to the CMS measure and requires either an intervention or extensive charting as to why the measure was not followed. Along with the hard stop notifications, data is drawn out of the EHR around such areas as mortality rates, surgical site infections, falls and other adverse events. This data is then analyzed and changes are made to improve processes in order to prevent future events. Root cause analysis is performed and changes are instituted as necessary to improve outcomes and meet the goals. Currently the facility is in-line with the national average for payment for heart failure patients, pneumonia patients and heart attack patients. Improvement is needed in the area of patient satisfaction scores and patient communication. An overall rating of one star indicates that there is a need for improvement.

Department One: Nursing (Patient experience of care domain – Communication) My rationale for choosing nursing is that nurses are the frontline care takers. They are the liaison between the patient and the physician and spend the most time with the patient during their hospitalization. B2. Goal. B7 Timeline The goal and timeline for this measure will be as follows: Engage nurses to work with other members of the healthcare team to improve the safety and reliability of care provided by developing a standardized handoff procedure that incorporates multiple disciplines and is patient centered. By year two completion, 75 percent of the hospital staff will have incorporated the new process of handoff and report and will meet protocol guidelines By year three, 100 percent of the departments will participate in and follow the protocols for standardized handoff.
