EXECUTIVE SUMMARY 2 Executive Summary Lee Memorial Hospital actively participates in CMS reimbursements and has emphasized the importance of Value Based Purchasing (VBP). In order to continue to strive to improve healthcare delivery to the residents of Lee County, Florida, additional initiatives must be implemented. Three domains within VBP have been chosen to add to the organizations strategic plan. Communication within the nursing staff will increase continuity of care and is a significant source for improvement of HCAHPS scores. The Operating Room remains the first source of potential surgical site infections (SSI). Therefore, it is important to address prophylactic pre- operative antibiotic administration in order to decrease incidence of SSI, decrease prolonged hospital stays and improve patient satisfaction and outcomes. Finally, in order to address morbidity and mortality rates of our patients, we are uniting the nursing staff, hospital based physician and primary care physicians to develop state of the art clinical pathways for follow up care and continuity of care for our patients as they are discharged from the hospital. With improved clinical pathways and directed, monitored care of our post-operative patients, we are hoping to increase the level of postoperative care to our patients and decrease complications and re-admission rates.
