Every healthcare system especially those who are adequately funded that providehigh quality care need to be supported by employees who can meet the growing population healthcare demands. In the health, continuum nurses consist of over 75% of the total healthcare workforce and play an integral role in providing healthcare. Even with the role, the nurses play inthe healthcare system they are not considered as equal partners with other multidisciplinary healthcare teams. Many programs including the ACOs and the medical homes are often based on the nursing scope of practice. Through the Affordable Care Act, it has provided an opportunity for the nurses to improve their distribution of care and developing essential mechanisms of care delivery. Continuum of CareNurses usually cover a more comprehensive continuum regarding the care they distribute to the patients. These services include disease prevention, promotion of health, coordination of healthcare to patients, and providing cure for various ailment together with palliative care in situations where treatment is impossible. This continuum of care is essential especially in the current and future need of the population. Many of the care the patients usually receive is provided by the nurses as they provide the majority of the assessments, evaluating a patient, and providing care in the hospitals, nursing homes, workplaces, schools and in an ambulatory setting
HEALTH MEDICAL 3(Sullivan, 2013). Nurses are also mandated to ensure that the care delivered to the patients is safe, compassionate, and effective. Nurses in their practice also handle patients and their family members in a holistic approach which may be depended upon the social, mental, and spiritual needs of the patient. Regarding the transformation of the healthcare system in general through the nurses applying their knowledge, creativity, experience, and providing adequate medical careto the patients, it will help towards fostering transformation in the healthcare industry.Accountable Care OrganizationsThrough the ACOs it ensures that nurses work as a team and provide care to a group of patients through high-quality, patient-centered and reduced cost care.
