NRSG 667-NRSG 665A: Acute/Episodic/Focused SOAP NoteChief Complaint: “I have a swelling on my big toe."SUBJECTIVEHPI: MK is a 58-year-old Caucasian man who presents to the clinic reporting of severe pain in his right foot big toe. He reports that he has been experiencing migratory pain in his wrists, ankleand now toes. He is currently experiencing severe pain on a scale of 8 out of 10. Most of the times the pain is exacerbated by an increase in physical activity and resolves after he has rested. Pain is also alleviated by taking diclofenac. Allergies to Medication: NKDAMedications: Diclofenac 25mg (For pain management). The last dose was at 6.00 am Medical History:He has been healthy until new chronic pain. Surgical History:No previous surgeries performed on MKHealth Maintenance:MK has attended all immunization procedures and is up to date.Family History:Father had a history of osteoarthritis. Personal and Social History: MK is happily married with four children. He lives with his wife and the two younger daughters. His sons are both working. He drinks at least three beers daily,
