Because of the explosion of the tail-mounted No.2 engine of United Airline Flight 232, the stage 1 fan rotor assembly parts were forcefully disconnected from the main engine. This resulted in the loss of the three hydraulic systems that were used for powering flight controls of the aircraft. This ultimately lead to the loss of control over the aircraft itself (Gonzales, 2017). After further investigation, it was soon discovered that the root of the accident was partly due to United Airline’s lack of consideration over human factors limitation in quality control and limitations procedures within the engine in the overhaul facility during maintenance. Because of this seemingly small oversight, failure of detecting fatigue crack emerged because of a formerly unidentified metallurgical deficiency which was located in a critical area of the stage 1 fan disk.
