MEDICATION ERRORS IN HEALTH CARE SYSTEMS 2 MEDICATION ERRORS IN HEALTH CARE SYSTEMS Introduction According to Paul & Elder (2006), the first step of the Socratic problem-solving approach says, “identify the elements of the problem, issues or question.” The concept is greatly applicable in analyzing the criticality of medication errors in health care organizations/systems. Makary & Daniel (2016) suggests that in most of the American healthcare facilities such as hospitals, medication errors has become the most common ethical problem or issue. Medication error refers to a preventable failure in patient's treatment process that might result or has the capability of causing harm. To analyze the problem of medication errors, the problem should be broken down into several elements. Breaking down a problem into simpler elements is a concept proposed by the second premise of the Socratic problem-solving approach. Such subdivisions of medication errors include causes, impacts and the solutions to medication errors in a healthcare setting. Elements of Medication Errors Karavasiliadou & Athanasakis (2014) claims that medication errors are preventable events and practices in the healthcare systems among the professionals. The concept of medication errors revolve around the provision of primary care services by the healthcare professionals. When non-safer primary care services are offered to the patients, medication errors occur. Health services across the universe struggles to offer quality care service to the clients and help to acquire their well-being. The primary care services are significantly critical in integrating the patient-oriented health care systems. These primary health care services offer an entry point

Accessible, affordable and safe primary care is significantly important to acquiring world health coverage as well as supporting the healthy lives and enhancing peoples’ well-being. When the services provided by the healthcare professionals fail to meet these care goals, it means that there was some medication errors in the process of treatment. Medication errors take place when the health care services provided causes inadvertent harm to people. Unsafe health care services are the contemporary primary challenges facing the world today. The researchers and health care scholars has done tremendous effort of understanding the causes, effects as well as possible solutions to the medication errors problem.
