I am going to be interviewing you for one hour. An hour is a very short time to get to know someone and so I apologize in advance if I need to interrupt you to move things along at times. First, let me ask a few general questions to help me to get to know you. 2. age How old are you? 3. sex Are you married? 4. Job What do you do for a living? What was your last job? 5. in-outpatient Are you an inpatient in the hospital or an outpatient. 1 and 2: Presenting complaint(s) and History of Presenting Complaint(s) A. HPC (2 + 2 + 2) A. Symptoms (2) 1. Can you start by telling me why you are in treatment. Course? 2. Relevant systems review: (specific to what tells you) MAP: mood (Depression, Elation) anxiety, psychosis 1. The problems you’ve been telling me about sound very difficult. I wondered if you had an idea what brought them on? What was going on in your life at the time things started to get worse? Were you under a lot of stress at work or at home when things started to get worse? Living circumstances?
