Shannon Stuckert NURS 6512 Wk 1 Discussion Initial Post Building a Health History As providers it our responsibility to build trusting relationships with each patient and treat them as individuals. One way to accomplish a good rapport with patients is to build a good foundation. This foundation helps us understand the patients better and build their care around them as an individual. The foundation begins with a health history. A history is to identify issues that the patient sees as a problem (Ball, Dains, Flynn, Solomon, & Stewart, 2015). The patient is a 76 year old Black/African American, male with disabilities living in an urban setting. Interview Making sure the patient is comfortable and you are sitting at eye level with the patient will help the patient feel more at ease during the collection of information (Lappen, 2011). First, asking a mixture of open-ended and closed- ended questions can help find focus as to what direction the health history needs to go to help achieve the goal.
