Introduction
The patients affected by cardiac diseases are, in most cases, under the risk of these
diseases and conditions affecting them anytime. Those at risk range from school children to
adults. According to Polit, & Beck (2013). , the nursing field is responsible for obtaining the
best approach to get a consistent and efficient measure to ensure patients are continuously at
their best health. The purpose of this study is to evaluate whether the patients affected by
cardiac diseases are provided for with home healthcare services if they are bound to have less
hospital readmission as compared to other patients who are not covered by these home-based
health services.
Background and Significance of Problem
According to Polit, & Beck, (2013), the problem was that the many patients who are
affected by cardiac diseases such as heart disease and lung cancer tend to be readmitted to the
hospitals for their condition either worsening or returning to harm the patients. Significant
reduction in the number of readmissions of these patients was noted when the home
healthcare services were integrated into plan. Statistics provided that with the involvement of
these health services, the number of patients being readmitted to the hospitals reduced.
However, more of the patients who were not covered by, the home-based hospital care
services were found to have a higher rate of readmission within the hospitals. As a result, it
was paramount to study and identify the approaches used by the home-based healthcare
services that were helpful in reducing the readmission of patients affected by cardiac
diseases.
The situation is of vital significance since these cardiac disease patients are comprised
of some persons from different settings. These patients include the students who are

continuing with their studies in various levels of education and within various learning
institutions. The victims also include the employees who are bound by their agreement to
work and be compensated for the number of hours or rate of productivity, which is reduced
by their readmission to the hospitals. Moreover, the employers who are responsible for
managing and directing their businesses and workers are also included in the category, which,
therefore, limits the number of profits these businesspersons have. Parents and children are
also among the population that is affected by such diseases, and with the readmission of these
patients being high, the families face the challenge of financial crisis especially if the
breadwinner is the affected party.
A more important aspect of consideration is the infections that these patients may get
while they are readmitted to the hospitals. With their health weakening, it is evident that more
diseases are likely to affect them. As such, the problem is significant and has detrimental
