Chapter 5 (Colbert) Bronchoconstriction – dec. in the diameter of the airway; 4 mechanisms o Bronchospasm – actual spasm or constriction of the smooth muscle of the bronchial o Airway edema – occurs when insult or injury to the mucous membranes causes dilation of the blood vessels and accumulation of fluid in the tissues o Secretions – reduce airway diameter; often the result of impairment of the normal mucociliary clearance mechanisms of the lungs o Inflammation – localized heat, redness, swelling, and pain as a result of irritation, injury, or infection Receptor Location Action Alpha Arteries and veins Vasoconstriction Beta1 Heart Inc. rate and contraction Beta2 Lungs and skeletal muscle Smooth muscle relaxation Bronchodilators – 3 categories based on mechanism of action o Sympathomimetics – cause bronchodilation by directly inc. cyclic adenosine monophosphate (cyclic AMP) o Anticholinergics – block effects of parasympathetic system by decreasing cyclic guanosine monophosphate (cyclic GMP), a substance that causes bronchial smooth muscle constriction o Methylxanthines Catecholamines – oldest group of inhaled bronchodilators o Effective and potent bronchodilators o Rapid onset and reach peak effect quickly o Short duration of action (0.5 to 3 hrs) o Require frequent dosing o Beta 1 and beta 2 effects o Inactivated by light o Can’t be taken orally
