Young man complaints of chronic case of back pain >3 months that is worse in the upper
back, joint pain keeps him awake at night, generalized symptoms includes low grade
fever and fatigue
? Ankylosing spondylitis (AKA bamboo spine) (chronic inflammatory
disorder that affects mainly the spine, improved with exercise but not with rest). Refer
to Rheumatoloigst , NSAIDs.
Kava-kava use
? Insomnia and anxiety
Bulge in bicep what imaging study you would do?
MRI is diagnostic study and hook test
to check for bicep tendon rupture.
Impetigo tx
- Muciprocin (Bactroban) ointment. If severe Keflex, Dicloxacillin,
clindamycin.
Mom brings 4 month old and c/o strabismus (cross eye) what do you tell her?
Normal up
to 6 months.
Hyperthyroid presentation and tx?
PTU (Propylthiouracil), Methimazole
What is the treatment for chronic bronchitis? Treat with SABA and inhaled
anticholinergics (Ipratropium).
Fingernail hematoma treatment? Trephination (make hole in nail through drilling or
piercing and allow blood to drain).
Characteristics of Hashimoto’s disease? Hypothyroidism s/s includes depression,
enlarged thyroid, fatigue, joint stiffness, puffy eyes, weight gain. Checks TPO (Thyroid
peroxidase antibody) confirm hashimoto’s thyroiditis (most common cause of
hypothyroidism). Check for other autoimmune disorders such as RA.
Risk factors for ectopic pregnancy? Previous ectopic, tubal surgery, current IUD use, hx
of PID and previous cervicitis.
A postpartum breastfeeding mom presents with dry, painful, burning and itching nipples.
What is the likely dx?

Osgood-Schlatter presentation?
Pain, tenderness, and swelling at the anterior tibial
tuberosity of knee. Most common in physically active males during rapid growth
spurts. Tx is RICE, NSAIDS and Tylenol PRN.
How to treat Chlamydia in pregnant women?
Azithromycin 1g /po single dose or
Amoxicillin 500 mg tid po x 7 days. Test for cure 3 weeks after completion of
treatment.
Ortolani and barlow are screening tests for ?

What are the consequences of untreated gonorrhea infection?
Women- PID, abscess,
ectopic pregnancy, infertility, can pass to baby during delivery. In men- epididymitis
and infertility. Both- it can spread to blood and cause disseminated gonococal
infection which is characterized by arthritis, tenosynovitis, and/or dermatitis and may
be life threatening, increase risk for HIV.
