This guide is intended tobe a summary of material modification and is an addendum to the Your Employee Benefits book or Summary Plan Description. You can also request a copy by contacting the FedEx Benefits Center at1.800.888.5622.For additional detailed information on benefits, exclusions and eligibility, go to the Choose Well Hub atchoosewell.fedex.com.An individual who is classified as an independent contractor or leased employee is not eligible to participate in any benefit planssponsored by the employer.FedEx reserves the right to amend or terminate any of the plans for any reason at any time.As a participant in the FedEx Corporation Group Health Plan, it is your sole responsibility to manage the benefits for you and any eligibledependents. This includes but is not limited to:• Making benefit elections by the required deadline,• Making sure that in-network providers are used in order to receive in-network benefits,• Timely reporting of qualified family status events online or by calling the FedEx Benefits Center, and• Providing any necessary documents within the time frame required by the plan.Falsifying information relative to your benefit coverage may result in termination of coverage.While the future of the Patient Protection and Affordable Care Act (PPACA) may be uncertain, our commitment to your health and well-being is not.
