The Office of Disease Prevention and Health Promotion developed in 2010 the HealthyPeople 2020 initiative program. According to ODPHP, one of the key national objectives isto “support behavior and or lifestyle changes” to improve healthy living as well as extendlife expectancy for individuals with health disparities that historically would have shorterlife spans.Moreover, since my graduate specialization is a family nurse practitioner programmy career goal is to work in an urban health center. Therefore, one coaching activity that Iwill do in this advanced practice role will be to provide educational surveillance. Thiscoaching activity will be in an effort to ensure compliance with my patients. Many of theindividuals who live in the urban community belong to racial or ethnic groups with thefollowing health disparities: uncontrolled high blood pressure, Diabetes Mellitus type II,elevated cholesterol and congestive heart failure.
