Presents to the clinic for a routine physical assessment and is agreeable to comprehensive health assessment. Patient denies pain or any health concerns. Has no known allergies, and currently does not take any prescription or over the counter medication. Received routine well-child care and is up to date with immunization. Received Gardasil vaccine at the age 14. Tetanus upon high school entry in 2014. Flu vaccine is not indicated as it is out of season. Family history includes mother and father alive and well. Mother has hypertension and father is a smoker. There is a second hand smoke exposure. Lives with parents in a rural community, is looking forward to graduationfrom high school this month has enlisted in Army National Guard, and leaves for basic training one month after graduations.
