3/3/2017 3 UNIPOLAR DEPRESSION Five main areas of functioning may be affected: Emotional symptoms Motivational symptoms Physiological & Behavioral symptoms Cognitive symptoms Somatic symptoms Symptoms may differ dramatically from person to person SYMPTOMS Emotional Sadness, depressed mood, irritability, anhedonia , crying spells, anger, anxiety, agitated, feeling ‘miserable’, ‘empty’, ‘humiliated’ Motivational ‘Paralysis of will’: social, occupational, personal areas Losing desire to pursue usual acities Lack drive, initiative acts, spontaneity Physiological & Behavioral Less active, less productive Sleep and/or appetite disturbances, psychomotor retardation/agitation , catatonia, fatigue Cognitive Poor concentration, indecisiveness, poor self-esteem, hopelessness, suicidal thoughts Somatic Headaches, digestive problems, dizzy spells, general pain UNIPOLAR DEPRESSION Two diagnoses to consider: 1. Major depressive disorder Criteria 1 – 2 – 3 are met Criteria 1: Major depressive episode Marked by five or more symptoms lasting two or more weeks In extreme cases, symptoms are psychotic, including Hallucinations Delusions Criteria 2: No history of mania Criteria 3: Symptoms must be severe enough to interfere with the daily functioning DIAGNOSING

3/3/2017 4 UNIPOLAR DEPRESSION Two diagnoses to consider: 1. Major depressive disorder Major depression, single episode => only one depressive episode Major depression, recurrent => 2 or more episodes separated by at least 2 months without symptoms Major depression, chronic => If MDD continues more than 2 years DIAGNOSING UNIPOLAR DEPRESSION Two diagnoses to consider: 2. Dysthymic disorder Depressed mood + two of: (a) poor appetite or overeating (b) insomnia or hypersomnia (c) low energy or fatigue (d) low self-esteem (e) poor concentration or difficulty making decisions (f) feelings of hopelessness Consistent symptoms for at least two years Never have been without the symptoms for longer than 2-month period DIAGNOSING UNIPOLAR DEPRESSION Two diagnoses to consider: 2. Dysthymic disorder When dysthymic disorder leads to major depressive episodes , the sequence is called double depression DIAGNOSING
