Analysis of Comprehensive Health Assessment
Mr. J. B. is a Caucasian 49 year-old male who resides in Fort Wayne, Indiana. He
presents for a comprehensive health examination and he is alert, oriented, cooperative, and calm.
His past medical history includes a dislocated right shoulder in 2016 and bilateral bunionettes for
which he is not currently being treated. He does complain of left lateral foot pain. He rates his
pain on a scale of 0-10, 10 being the worst as a 2 with rest and 3-4 with activity. He states, “It’s a
dull ache, especially when I am on my feet all day. I can really notice the ache while driving
home after work.” Mr. B. states the discomfort increases with running and when it is extreme he
switches his activity to using the elliptical or biking. He denies using NSAIDs to help alleviate
his pain. He reports no history of allergies to medications or environmental and he reports taking
no current medications. He states he has had all childhood immunizations, but refused the
influenza vaccine. Family history includes his Mother, 68, who is still living and has recently
undergone cataract surgery on her both eyes. She recently started wearing bilateral hearing aids.
She is currently taking medication for hypertension. His Father, 68, is also living and recently
undergone cataract surgery as well and also is taking medication for hypertension and
hypercholesterolemia. He has a younger sister who is alive and without disease. He currently
lives with his wife of twenty-three years. They have two young adult sons both still living at
home. He has a Bachelor’s in Education and has been employed as a school teacher for the same
school system for the last twenty-one years. He drives a total of 100 miles per day to get to and
from work. He states he does not have a living will or advanced directive at this time. He states
he tries to eat healthy, but enjoys an occasional snack such as ice cream or chips. He enjoys
running, biking and walking their dog. He also enjoys spending time with family and friends and
