Running head: COMPREHENSIVE HEALTH ASSESSMENT FOR PATIENTS AND POPULATIONS 2 Comprehensive Health Assessment for Patients and Populations Interviewing techniques Social determinants of health are conditions in the environments in which people are born, live, learn, work, play, worship, and age that affect a wide range of health, functioning, and quality-of-life outcomes and risks (Healthy People, 2020). The patient’s overall appearance, their body language, and their responses during a comprehensive health assessment can help the provider assessing the patient determine their SDOH and help establish a better understanding of the social status of the patient. It is important to obtain a patient’s social determinants during a health assessment to determine their ability to maintain a healthy life style, their access to health care, and their ability pay for health care. It is also important for determining their understanding of healthcare and disease prevention as well as the resources available to them. This allows for the interviewer to have a better understanding of any adherence or noncompliance issues. Questions were presented in a face-to-face format using a private setting to decrease interruptions so the patient would feel comfortable expressing themselves more freely. Before the assessment began, social determinants, such as access to health care, affordability of health care, and maintaining a healthy life style were explained to the patient and the importance SDOH has on their overall health and well-being. A patient-centered approach was used to help develop a rapport and to assure the patient the interviewer has their best interest at heart. The interviewer used a variety of guided questioning including open-ended and focused questions to help the patient feel comfortable and willing to share more of their story. The interviewer also used the technique of actively listening. This is done by maintaining eye contact with the speaker, putting aside distracting thoughts, avoiding formulating responses

By utilizing active listening and empathetic responses, the interviewer was able to build a trusting rapport with the patient and show support. This allowed for the patient to communicate more openly and express feelings they may not normally share. Throughout the comprehensive health assessment, the interviewer used summarization. Summarizing the patient’s story throughout the assessment helps to communicate to the patient the interviewer has been listening and to clarify any misunderstandings. Through the comprehensive health assessment, the interviewer was able to establish economic status, education level and employment, community involvement and access to health care.
