LEADERSHIP EXPERIENCEA1. Problem or IssueThe current issue on my unit is the lack of hand hygiene. It can be challenging to perform properhand hygiene when wall hand sanitizer is not full. This current problem aligns with the organization’s priorities of hand hygiene policy to reduce hospital-acquired infections (HAIs).A1a Explanation of Problem or IssueHospital-acquired infections (HAI) can lead to higher health care cost, extended stay and increase mortality rates (Haverstick, 20170. Hospital-acquired infection affects 1 in 25 patients in the acute setting. Hand hygiene education is essential to reduce and prevent HAIs. Performing hand hygiene is an integral part of hospital infection control. On the pediatric unit, we have patients in isolation in one room, or the next room has a newborn infant. On a typical day, we may have 1-4 patient continually going in and out of each room. Without proper hand hygiene, exposure to infections can occur. InvestigationI investigated the problem through observation and interview of staff. I observed the staff limitation to access soap and water. The unit has one sink at the end of the hall. I saw nurses not performing hand hygiene before entering the room. Other nurse tried to use the mounted hand sanitizer outside the room. Many time it was empty. I asked nurses why they did not perform hand hygiene? Response “I am too busy and too busy to call housing keeping refilling hand sanitizer.” I also observed some nurses who carried a small hand sanitizer they used before entering the patient room. Currently, the hand hygiene compliance results for the last three quarters are: 1st quarter 94% 2nd quarter 94% and 3rd quarter was 94%. In addition to this audit the hospital as a whole rank 91% compliance regarding hand hygiene. Hand hygiene compliance needs to improve. 2

The standard mode of transmission is via contaminated hands. The best method to prevent infections is for staff to perform hand hygiene (Haverstick, 2017). A3. Taking care of the 3-4 pediatric patient in isolation and having to gown up to go into each room.A3.
