Asked the patient to rate his or her pain at the wound site. Applied clean gloves. Removed the old dressing, and assessed it for exudate and drainage. Folded the soiled sides of the dressing together, and disposed of it in the appropriate receptacle. Removed gloves and applied sterile gloves for wound assessment. Assessed the wound by looking for swelling, separation at the edges of the wound, inflammation, and drainage.
