THE NURSING PROCESS Objective data are data about a patient that the nurse observes directly. The nurse can check body temperature and see drainage and swelling; hence, they are examples of objective data. Signs and symptoms presented by a patient, but which the nurse cannot observe directly, may be significant and are referred to as subjective data. For example, headache and nausea are symptoms that a patient may experience but the nurse cannot see. A verbal description of the patient’s health is considered subjective data. These may include feelings, perceptions, and self-report of symptoms. Therefore, complaints of nausea and tingling sensations are considered subjective data. Objective data are the measurements of a patient’s health status. Therefore, measurements of blood pressure, radial pulse, and fasting blood sugar levels are considered objective data. Probing explores the history of the patient without controlling the direction of the story and is meant to bring out as much information as the patient can give. A leading question is meant to
