* enters room* * lays out materials and washes hands * “Hi my name is taylor and i will be your nurse today, i'm just going to be checking your vitals and doing a quick head to toe assessment, any questions before we start?” “No” “Great, so, can you confirm your name and date of birth for me” “Alec durkin, 12,23,1997” “Great, i’m just going to start with your blood pressure is it okay if i touch you?” “Yes” “Do you have a preference of what arm i use? “No” * Begin to get ready to take BP* “If you were to have any trauma or and IV in a particular arm I would use the other” * look for brachial pulse, once brachial pulse is found place cuff one inch above, pump up until you don’t feel radial pulse anymore then pump 20+, slowly release first beat is top number, last beat is bottom number* “Okay your blood pressure is ____ over ____, now i’m going to take your apical pulse, could you lay back for me” *lays down* *take apical pulse at base of heart for one minute, then look at chest and take respirations for 30 sec* “So your apical pulse was ____ and your respiratory rate was _____, now i going to take your temperature… say AH” *takes oral temperature*
