Initial Assessment of Exercise, Lifestyle, Behaviors and Attitudes Answer questions in point form EXERCISE/NEEDS 1. How many times a week do you exercise? (Running for a bus and walking up the stairs in the N building does not count) 2. How many times a week do you weight train? 3. How many times a week do you do a minimum of 20 minutes of cardiovascular exercise? 4. How many times a week do you play sports with friends? I.e. soccer, volleyball, racquetball etc.. 5. Do you have any injuries? If so, what are they? List your Scores and Verbal Ratings 12 minute run - # of laps (Ignore verbal rating for this one) Chair Dips - Push ups - Curl ups - Planks - Wall Squats - Waist Circumference - Hip Circumference - Waist to Hip Ratio - LIFESTYLE/BEHAVIORS 6. If you had to estimate, how many hours a day of “screen time” do you engage in? 7. Do you feel better or worse after spending time on social media? I.e. Instagram, Facebook, Twitter, Snapchat, explain your answer. 8. Do you sleep with your phone under your pillow? If yes, why?
