Building a Comprehensive Health History Mr. D a 76-year-old African American male with disabilities living on the 3rd floor in Assisted Living in Washington DC. The client utilizes public transportation for errands and physician visits. Assessment Techniques during client’s visit. Obtaining a good history, including information on social circumstances and lifestyle in addition to medical and family history is crucial to good health care. As the appointment begins, the provider uses open-ended questions, “How are you?” and “What concerns or symptoms would you like to discuss in today’s appointment?” or “What brings you into the clinic today?”. This initiates dialogue and establishes trust between the client and provider. Asking Mr. D. evaluates his understanding of his disabilities, health issues, and how he controls and manages. Open-ended questions encourage the client to discuss, his concerns regarding his healthcare. The provider should be cognizant, fully engaged and be aware of verbal and nonverbal gestures. Refrain from discussions resulting in “yes” or “no.” answers from the provider. Permit the client time to answer questions, this allows for uninterrupted time to express concerns.
