Katherine Wolff Case Discussion AH231 On July 18 th -Mrs Jones records were copied an sent to her by Ready Chart. I believe this was a breach of confidentiality by the hospital. Records must be kept private by the HIM. By allowing third-party access to Mrs. Jones records and copying them to send to Mrs. Jones anyone could have read her information and used it to their advantage. A third party should not have been used. The HIM should have had Mrs. Jones sign a release and verified who she was, then either sent to her electronically, sent a paper form or CD to Mrs. Jones. On October 5-Nurse at Mercy Hospital requested records to be sent to their ER by fax. Mrs. Jones records should have been sent via encryption (electronically sent to the other hospital.) By Faxing records from St John Hospital to Mercy Hospital Mrs. Jones records were again subjected to prying eyes. All info should be sent by the safer route (encrypted email.) On October 15 Mrs. Jones calls St John’s Hospital HIM department to ask that records be sent to another physician (Dr. Lyon.) Mrs. Jones would have been verified by the HIM department, asked to sign an authorization form to send the records to another physician electronically or by mail. (Preferably via CD or
