Proposed Solution
The author will propose a project to implement changes in patient monitoring to identify
respiratory depression in patients receiving opioid narcotics on the 5 South unit at Kaiser
Permanente Fontana Medical Center. The proposal is to add capnography end-tidal CO2
monitoring for all patients that have intravenous opioid narcotics ordered on 5 South. However,
pulse oximetry will not be discontinued during the initial implementation of capnography for
liability reasons and patient safety (Schmidt & Brown, 2015). This proposal will include a test of
author stated earlier, research sited in this paper show that capnography can reduce opioid-
induced respiratory failure (OIRD) (Stites, Surprise, McNiel, Northrup, & DeRuyter, 2017).
The current setting for this project is realistic and appropriate. There have been random
patients that capnography has been ordered for on occasion on this unit. Nurses have reported
concerns regarding lack of consistent training in using capnography on 5 South. The training and
education concerns of the nurses are being addressed in this project and will be discussed in the
implementation plan.
