Field Placement Midpoint Progress Report Student Name: Site Preceptor Name: Faculty Advisor Name: Date and Time of Midpoint Review: Instructions for Students, Site Preceptors, and Faculty Advisors The student, Site Preceptor and Faculty Advisor should convene by conference call or in person to conduct a midpoint review approximately halfway through the student’s Field Placement. If the student is enrolled in Field Placement over two semesters, this review should take place at the end of the first semester. In preparation for the meeting, the student will complete this report and email it to the Site Preceptor, Faculty Advisor, and Program Coordinator. During the conference, this form may act as a general outline for discussion. After the meeting, the student will revise this report based on the discussion, and s/he will email the revised version to the Site Preceptor, Faculty Advisor, and MPH Program for review and approval.
