Client reports a 30-year history of crack cocaine use. He has been to treatment before and has been able to maintain as long as 2.5 years sobriety. Client says that relationships and money cause him to relapse. Client is currently engaged in a 12-step group and has a sponsor. He is able to return to his current employer when able to work and is willing to move into sober living. Client has completed a coordinated assessment. Client denies any mental health diagnosis and says he does not want to take psychiatric meds. He is working with a Pretrial Services caseworker and requested that counselor contact his caseworker and his lawyer regarding his intake and participation in the ANE program. Client's upcoming court date is regarding a recent family violence charge. Client would like to work things out with his significant other but says that she has addiction issues as well and they need to focus on themselves currently. Client became tearful when talking about her. Client has no local family support.
