Client was very talkative, cooperative and answered questions about his alcoholism which began at the age of 19 and his cocaine use began at the age of 26. Client appeared to be happy to be at ANE to start rehabilitation for his alcohol addiction. Client stated he is ready to stop drinking because he has messed up a lot of relationships and is ready for a change. Clients stated that he has gone 3 or 4 months without drinking but relapses when he becomes involved with a new woman who uses drugs/alcohol or people who are drinking and using drugs which is a trigger for him. Client thought about committing suicide in 2016 by jumping in front of a truck but didn't go through with it. Client has been in the hospital for a mental evaluation and was diagnosed with PTSD, Bipolar disorder and schizophrenic effective disorder. Client is taking Harvoni for Hep C, Metformin & Glyburide for type 2 diabetes, and Lisinopril for hypertension. Client states parents didn't drink or do drugs however his brothers and sisters drank and used drugs. Client stated he was devastated due to the death of his favorite sister in 2018 and that he drank and used drugs more often to drown his sorrow of losing his sister.
