Chief complaint patient states she is nervous prior to going to sleep which makes sleeping difficult. Patient states onset of symptoms were 3.5 weeks ago. Patient states this experience happens 4-5 nights a week especially after reading for a long period of time. Patient states when she feels anxious she feels on edge and nervous and her thoughts starts race and she starts to think about all the things she has to worry about. Patient states she has had an increased amount of stress lately that's making it harder to sleep. Patient states she has been tired a majority of the day and finds it hard to focus especially when she has to study for an exam coming up. Relieving factors include distraction techniques. Patient states she usually tries to stay organized and make to-do list and plan her time accordingly but this has not been helping as of recent. Patient denies treatment. Current medications Flovent two puffs daily albuterol two puffs Q4 hours PRN for shortness of breath Allergies penicillin causes runny nose itching and sneezing just causes runny nose sneezing and breathing problems cats cause runny nose itching and sneezing Social history: patient is a single African American female who is a supervisor at Mid American ship she's pursuing her bachelor's degree in accounting. Patient states she watches a large amount of TV throughout the day usually watches marathons of episodes states she also bought an e-book reader and that her and her friends want to start a book club. States she also attends church for free talks. Patient denies legal history or military service. Patient states uses their seatbelt when in the car. Patient states she lives at home with her mother and sisters has her own bedroom and they share a kitchen station will be attempting to get her own place when she finishes College. Patient describes a well-organized family HPI: Ms. Jones presents to the clinic complaining of difficulty sleeping which she notes to have started 1 month ago. She states that her sleep is “shallow and not restful”. She complains of difficulty falling asleep
