IMPROVING SAFETY AND CARE2The Institute of Medicine (IOM) has defined patient safety as “the prevention of harm to patients or freedom from accidental or preventable injuries produced by medical care” (Mitchell, 2008). A high quality health care delivery system is one that is focused on preventing errors, hires healthcare providers that learn from their errors, and is built on a culture of safety. Nurses play a large role in building and maintaining this culture of safety.New Practice ApproachIt has been recommended that nurses become leaders of interprofessional teams in improving health care systems and patient care, this has led to a focus on nurses utilizing evidence based knowledge in clinical decision making and using evidence based research for interventions (Stevens, 2013). This practice is called Evidence based practice (EBP) and it is when a health care provider combines the best research evidence with clinical skill and patient values to provide the best quality care. Utilizing EBP in a health care facility requires several phases. The first phase is the research phase; research needs to be gathered and put together in a way that shows that it will be functional to nursing practice. The second phase is presenting the research to stakeholders; to develop a project plan. The third phase is implementing the research project and the fourth phase is implementing the evidence based practice. This students proposed evidence based practice project is a research study on how improving interprofessional communication and teamwork with daily huddles and standardized communication tools can improve patient care and patient safety. This student found this research topic to be very interesting and one that will be useful for any nursing environment or area.Intraprofessional Collaboration
IMPROVING SAFETY AND CARE3It has become more apparent that for patient care to be safe and of the highest quality, health care providers need to have an approach that is collaborative, team-based (American Nurses Association, 2018). When nurses are able to work together as a team there is an improvement in communication which leads to a reduction in errors; this in turn leads to an improvement in patient safety, improved patient care and improved quality of care. Interprofessional communication is extremely important in health care which is the reason this student has chosen this topic for this evidence based practice project. Teamwork is necessary forthere to be an improvement in patient safety; two ways to improve teamwork are through daily huddles and the implementation of standardized communication tools.
