Acute Kidney Trauma Damage to the kidney caused by either blunt or penetrating injury (motor vehicle accident, assault, fall, sports injury, blast, knife, or gunshot wound) Grade 1: superficial laceration of the renal cortex, contusion of the parenchyma Grade 2: hematoma of the kidney that does not expand, laceration less than 1 cm of the parenchyma Grade 3: laceration 1 cm or more of the parenchyma, collecting system is intact Grade 4: thrombosis in a portion of the renal artery, bleeding of the renal artery and veins that is controllable, laceration 1 cm or more of the collecting system, leaking of extravascular fluid around and within the kidney Grade 5: fractured kidney, tearing of the renal artery or vein, thrombosis of the renal artery S/S Sign of wound Flank bruising Hematoma present Blue-red purple or green-brown flank discoloration (turner’s sign) Flank tenderness or pain Hematuria Blood clots in the urine Hypotension Hypovolemic shock (grade 5) Diagnostics
