The client and his family live in the Northeast area of Columbia, SC. His primary language is English. The client is in the 6 th grade at a local public school. He is insured with Medicaid. He is a Christian. His great-aunt is a lower-class factory worker, single, and just recently ended the relationship with her son’s father. Presenting Problem : “The client states “I get side-tracked in class and can’t stay focused, and everybody is always blaming me”. The client’s guardian states “he is disrespectful and never remember what his homework assignments are, his teachers are constantly reporting that his never completes assignments and is contently involved in verbal and physical altercations with his classmates”. History of Present Illness: This client comes to the clinic with his guardian (great-aunt) and her 5-year-old child for an assessment of the above complaint. His guardian has multiple concerns about his ability to stay focused and having healthy relationships with family and friends. She also states that he constantly tells her that he doesn’t like his teacher. Practicum Experience Time Log and Journal Template suspended for throwing scissors and a book across the room because the teacher wouldn’t allow him to go to the restroom. According to his guardian, the administrator calls her several times throughout the day about his classroom behavior and having to remove him from various classes, so the teacher can continue teaching. The teacher reports that his excessive talking and attention seeking behaviors are causing him to miss valuable school work. Past Psychiatric History: The client was diagnosed with Attention deficit hyperactivity disorder (ADHD) at age ten by his family physician and has not had any formal diagnosis before that. The client’s guardian reports that in the past six months, she has noticed that his behavior and grades has declined. He had an A in art, F in math and an F in English. ADHD is a chronic condition that disturbs the emotions, behaviors and daily learning mainly in children (Healthline, 2018). His guardian opted out of medication management at that time. With the recent school disruptions and drop in grades his, she felt the need to seek medication management, and the client is currently taking Concerta 27 mg. He has been on this medication for two weeks with little or no noticeable side effects.
