2A1. Patient PopulationThe Clinical Transition Unit (CTU), located at Florida Hospital Orlando, is a 65-bed environment where patients are admitted under observation status. The unit sees multisystem patients such as chest pain, abdominal pain, orthopedic patients, neurologic patients, and surgicalpatients. In addition to seeing multisystem patients, there is also a direct patient care clinic where patients receive outpatient services such as daily IV antibiotics, blood transfusions, and wound dressing changes, as well as a suture clinic which allows physicians to provide stitches and sutures when necessary to close and ensure the proper healing of certain wounds. The team is currently below benchmark performance measures on the unit and in efforts to make improvements, a balanced scorecard will be developed to address improvement in the following areas: performance, quality, patient safety, and employee engagement.A2. 3intellectB. Necessary ProcessesPerformance: The outcome/goal is to improve discharge time from the time the dischargeorder was given until the time the patient is discharged from the unit. The necessary processes that will be implemented include adding a discharge nurse for day shift and night shift. CTU is a rather large unit that is very fast-pace. A lot of times nurses become busy with critical patients and not realize a discharge order has been received for one of their other patients, resulting in a late discharge and the patient becoming upset. By adding a discharge nurse, it will improve patient satisfaction scores because the patient was discharged in a timely manner. Also, adding a discharge nurse will improve reimbursements as a result of a shorter length of stay. Additionally, there is a faster turnaround by adding a discharge nurse because it allows patients to be discharged and provides open beds for other patients. There is also a better use of staff with the addition of a discharge nurse because nurses are not overwhelmed when caring for a critical patient and under pressure to discharge another patient.
