VBP Degree of Implementation The Hospital Value-Based Purchasing (VBP) Program is a Centers for Medicare & Medicaid Services (CMS) initiative that rewards acute-care hospitals with incentive payments for the quality care provided to Medicare beneficiaries. Section 1886 of the Social Security Act established the Hospital VBP Program, affecting payment for inpatient stays in more than 3,000 hospitals across the country. The initiative is a part of CMS’s larger quality strategy to reform how health care is delivered and paid (Centers for Medicare & Medicaid Services, 2017). Florida Hospital is a hospital located in Orlando, Florida and has an overall rating of three out of five stars. The overall rating shows how well each hospital performs, on average, compared to other hospitals in the United States. The more stars, the better a hospital performed on the available quality measures (U.S. Center for Medicare and Medicaid Services Hospital Compare, n.d.). Florida Hospital has fully embraced the value-based purchasing (VBP) program through implementing an electronic medical record along with a clinical decision support tool that has been used to decrease patient length of stay as well as provide improved patient outcomes. Implementing a clinical decision support tool is very important because it builds upon the foundation of the electronic medical record and provides organized specific information to enhance health and healthcare. For example, there is a patient on the unit that is one-day post-op. When accessing the patient’s electronic medical record, the physician will receive a notification that requires their attention to remove the patient’s Foley catheter. As a result of this notification, the patient is at a decreased risk of developing a hospital-acquired infection, which is one of the outcome measures used in the hospital VBP program. Florida Hospital ranked Better than the National Benchmark in the category of Catheter-associated urinary tract infections (CAUTI) in ICUs and select wards . This is the direct result of the clinical decision support tool located

Nursing Department (Patient Experience Domain: Nurse Communication) The role and responsibilities of the nurse include being an educator, patient advocate, caregiver, and decision maker. The nursing department is important for implementing VBP in the organization because nurses are the liaison between the physician and the patient. Nurses are the primary caregivers and the actions of the nurse impact the outcomes of the patient as well as the measurable outcomes of the hospital VBP.
