Assess Pt is a 30 year old male seeing his HCP for annual physical required by his occupation as a truck driver. Ethnicity is Spanish- Puerto Rican. Religion is Christian/Catholic. Typical daily diet contains energy drinks, breakfast sandwiches, rice, and meat. Pt is married with two young children and lives at their own home. Pt admits to frequent use of smokeless tobacco and consumes about 15 standard beers each weekend. Nurse questions regarding culture: What is your ethnicity? What is your typical diet? What is your religion? Are there any dietary preferences/restraints related to your religion? Do you have children? What is your occupation? What are your hobbies? By asking theses questions, you can better understand the patients lifestyle and what matters to them.
