APPIER -Schizophrenia ASSESSMENT (What data would align with the admitting / medical diagnosis? Objective? Subjective? What lab or diagnostic tests / values are relevant to this patient?) Chief complaint, HPI, Psychiatric history, alcohol and/or drug use, medical history, family history, developmental/social history, culture, behavior, appearance, mood/affect, speech, thought process, content, impulse control, cognition/sensorium, any history of violence or aggression, perception of current situation, hallucinations, thoughts or attempts at suicide. PATIENT PROBLEM LIST (What are some potential patient problems [nursing diagnoses]? Rank these in order of importance.) Disturbed thought process: Disruption in cognitive operations and activities. Risk for other-Directed violence. Self-Care deficits PLAN / GOAL SETTING (What are some goals that could correct the problems identified above? Patient will…(SMART – specific, measurable, action-oriented, realistic, timed.) The client will: Be free from injury throughout shift Demonstrate decreased anxiety level by end of shift Respond to reality-based interactions initiated by others. For example: verbally interact with staff for 5-10 min by end of shift. IMPLEMENTATION (What will you (as the nurse) do to assist the patient in meeting the above stated goals and patient problem list? If it is a repeated intervention, please state the frequency in which you would conduct the intervention. Next to each intervention, identify which patient problem you are addressing.) Be consistent in setting expectations, enforcing rules and so forth. Interact with patient on basis of reality do not dwell on delusional material. Encourage the client to communicate without prying. Give positive feedback for the clients success. Be sincere and honest while communicating with the client. EVALUATION (How will you know the interventions were successful?) The client demonstrates a reduction in s/s of anxiety and apprehension. The client remained injury free throughout shift. The client was able to participate in a 5-10 minute, reality-based, conversation with staff.

RISK POTENTIAL: What is the worst possible complication this patient could experience? [Do not write death]; What assessments and interventions will address and potentially reduce the risk of this complication? Injury to self or others. Assess if the client has ever attempted or considered suicide. Determine if the patient has problems with aggression or violent tendencies. Assess for auditory hallucinations. Place the client within eyeshot of the nurses station for ease of observation.
