Psychology 170 – Class Notes – 2/19/09 Last time: Bipolar disorder and manic and depressive episodes Bipolar disorder – just described the disorder Treatment : Lithium VIDEO: Bipolar Disorder or Not? Sometimes people fit the diagnostic criteria exactly , but this RARELY happens. GOAL : UNDERSTAND what really does make sense in terms of her having bipolar disorder and what does not in other words is the diagnosis appropriate? DEPRESSIVE EPISODE: When you are taking the medicine, is there still an active battle between good and evil? Yes She feels nervous She hears voices in between the flare ups they stop when shes on the medicine First time – Sick 1985 1988 – God came to her 1990, 1992, 1993 in between those times, no voices In between the flare ups – she feels normal She hears voices She feels scared and anxious Periods of time when she felt bad about herself self-critical – fat and ugly?? o Felt depressed o She thinks her husband cheats on her When she was on the meds – She burned her arm She burned herself like she was burning her heart because her husband walked away from her

MANIC EPISODE: She is going to do whatever the doctor tells her she can do – example of some form of self-restraint She beat up her husband o He came to her house What is your mission? o “Fight for the American way” o Delusional – she thinks she is a secret agent for Jesus She thinks she can control the elements She likes the outdoors She likes gum She cries o Her mother died April 5 th , 1993 She wasn’t able to walk She had cancer in her backbone She has a hard time walking She gets drunk on: (drunk = euphoric high) o Coffee o Water o Kool-Aid Only when she is a period of mania She never feels “bad” during periods of mania: o She still has lots of energy, just as though she had a full night’s rest IN-CLASS WORKSHEET: 1. What did you notice in the woman’s behavior that is consistent with your understanding of depression/? a. Delusional – battle between good and evil, she thought she was a secret agent b. Crying – she was sad at times c. Self-critical (negative thoughts) i. Fat and ugly d. Self-mutilation i. She burned herself 1. As a suicide attempt, yes 2. Act of self-hatred, yes, a. BUT, without knowing more, we don’t know the real source of self-hatred e. She used drugs to get through the hard times f. Hallucinations – consistent with some forms of psychotic depression g. The depression was cyclical – it came and went h. Apathetic – emotional blunting – more common in schizophrenia though

NOTE : Mood congruent delusions go with the mood disorder (so if it’s bipolar disorder, the patient will have depressed delusions and hallucinations when he/she is depressed and will have really upbeat, positive, crazy delusions/hallucinations when he/she is in a manic (hypomanic) state 2.
