Problem: Patient’s receiving blood transfusions are often persecuted to interruption of sleep, hindered ability to ambulate or participate in unit activities. The inconsistencies that exist in recommendation for the frequency of vital sign monitoring during transfusion are costing patient’s their lives. In 2011, the United States Food and Drug administration (FDA) reported 69 transfusion fatalities, 30 of which were directly caused by the blood transfusion itself. With so many patients relying on blood transfusions we owe it to them to find better what is best practice. The facility in which where this study took place policy for blood transfusion assessment consisted of taking the vitals signs before the transfusion to record a baseline and then collecting vital signs again after 15 minutes of initiating the transfusion, then hourly throughout, and again 30 minutes after completion. Intervention: This article states that best practice guidelines suggests that less frequent vital sign monitoring may be appropriate if couples with thoughtful physiologic assessment. More
