A program logic model and flowchart would have been helpful to document each step of the service process. Activities should be documented in case files and additional records. During, intake a face sheet should have been developed, containing basic demographic data such as, age, race, school performance, and their involvement with additional services. Including any other information that may be needed. Client data could have been collected by using checklists or forms. There should also be a screening/assessment process to gather client information and create a case plan, including objectives, which should all be documented in the client file. Another form that could have been used for developing the case plan, would outline services and activities to be provided, including attendance at groups, and involvement in the topic. The case plan should have objectives and accomplishments that can be documented and measured. Also
