Unit 3: Discussion - Population Health Management 1 Unit 3: Discussion - Population Health Management There is a high incidence of diabetes in the community along with high frequency of outpatient doctor visits and hospital readmission due to diabetes. Based on the report diabetes still maintains in the early-stage identification and elimination of the disease. A population if health program is in order to prevent further progression of the disease. Development of a community health promotion program addressing the risks associated with diabetes Promoting the program will be done in various ways with social media, public health departments, insurance and outreach events. Addressing the risks will be done a professional manner, not to intimidate or scare anyone, but to raise awareness on how serious the disease is. Weight, inactivity and high blood pressure are risks linked to diabetes that can be prevented and managed through the program. Social Media: It has become a platform for promotion whether it may be for make-up, shoes, or a song, why not use it for healthcare. Through social media we can demonstrate physical activities that can be done at home, we can demonstrate step by step healthy recipes, and upcoming events.
