Identify a problem or issue related to practice, policy, population, or education that aligns with the organizational priorities you see to solve. Proposed initiative to “Lift the Linens” and both outgoing and incoming nurses access the site during bedside report; so as to raise the level of patient safety, enhance patient experience, and aid in efficient nursing practice. A1a: Explain the problem or issue, including why it is applicable to the area of practice you chose and the healthcare environment . The problem is site inspection of the patients’ wound, incision, colostomy, NG tube, cellulitis and more. is not done routinely during bedside handoff. Site inspection with both the incoming and outgoing nurses, patient/family is applicable to hospital direct patient care has an improved quality healthcare, patient safety and patient experience. A2: Discuss your investigation of the problem or issue. After random verbal polling of the regular full-time, night shift and PRN nurses on BMHCC Pre-Op/Post-Op it was discovered that overall both outgoing and incoming nurses could benefit form setting new team goals to inspect sites during report. These sites include, but not limited to are ostomies, Foley’s, NG tubes, PICC lines, drains, incisions, wounds and/or other devices. These nurses provided personal examples of how not doing this has impacted patient safety, patient care and/or nursing interventions. Examples stated, but not limited include: Non-emptied colostomies, that rupture/leak during or shortly after shift change NG tube dislodgement as a result of lose taping. NG tube left to count marked at 900 ml. Overfull and/or not compressed JP drains

TASK 1: Leadership Experience PAGE 3 Overdue Central Line dressing changes which could lead to infection Not surveying at shift change what supplies are in the room and rushing later during peak periods for; inner cannula, stoma past, wound vac supplies et. A2a: Provide evidence to substantiate the problem or issue (organizational assessment, national source documents, evidence form a stakeholder). A 20% base line was determined by verbal survey within 5 North Pre-Op/Post-Op Unit by myself (Nurse Quinn), and with the supervision of my Nurse Manager (Monique Cannon). During six-week period 60 stake holders of patients/families were surveyed. 12 stake holders indicated that both nurses made a joint physical assessment of the patient’s site(s) status during bedside handoff. A3. Analyze the state of the situation using current data.
