Safe 256 PBL #2 You are the safety manager of a chemical process facility that recently changed a process in order to facilitate a new customer’s large order. You realize that by doing so your company will now fall under the OSHA PSM standard. You know that there is an OSHA requirement that the hazard analysis team be made up of employees that are familiar with and have knowledge of the process. What possible methodologies could your hazard analysis team use, and how often would you have to update the analysis? Several methodologies could be used. Possibilities could be the what-if, or checklist, or what- if/checklist. I like the HAZOP (Hazard and Operability Study), or the FEMA method could be used, or the fault-tree analysis. The team could use an appropriate equivalent methodology.

Also, one member of the team must be knowledgeable in the specific process hazard analysis methodology being used.
