A Tale of Two Tourists – 1 A Tale of Two Tourists – 1 Scenario Two vacationing tourists, unknown to each other, visited the Alabama gulf coast over the same weekend in August and decided to take a swim in the Gulf of Mexico. One, a 12-year- old girl named Alexis, had a cut on her arm caused by a fall onto the sharp edge of a scooter. The other vacationer, Brad, a man 66 years of age, had recently cut his leg on a nail protruding from a wooden railing at his beach condo. Within moments of entering the Gulf of Mexico, several small Gram-negative microbes invaded both of their bodies through these cuts. Four days later, the young girl was heading back to Birmingham in the back of the family car, oblivious to the battle recently waged in her bloodstream. At the same time, Brad lay dead of an aggressive blood infection.

A Tale of Two Tourists – 2 A Tale of Two Tourists – 2 Brad’s Clinical Progression Within a day after his swim, Brad’s leg began to swell, was warm to the touch, and started to hurt. He put off going to the emergency department until day 2, when he realized that his leg was now extremely painful and had started to darken, and he felt feverish. At the hospital he had a fever of 39°C (102°F) and his blood pressure was abnormally low.

A Tale of Two Tourists – 3 A Tale of Two Tourists – 3 Brad’s Clinical Progression Samples of wound material containing pus and a blood culture were collected and sent to the diagnostic laboratory to determine the identity of the pathogen. Results of the tests, however, would not be known for at least 48 hours. Meanwhile, he was admitted to the hospital and placed on empirical antibiotic therapy to try to stop the infection.

His fever remained high and he became unresponsive. The antibiotics were not working. Brad died the next day. The lab report, now completed but too late to matter, indicated that both the wound and blood cultures contained Vibrio vulnificus .
