Sharmin Khatun Unit 07 Understand person-centred approaches in adult social care settings 1.1 Define person-centred Values Person centred values is when an individual who are able to be involved and included in every aspect of their care and support. For example: support planning, assessments etc. Policies and procedures and care practices should put the residents at the centre of the day to day Activites. Also it should include the residents and theirs in planning and maintain of this. A person centred is values. These values are in place so the individual is respected when you’re doing your job also to be supportive. The values protects the right of the customer and everyone refers backs to them so they are implemented to enable the customer that is being supported to be given Dignity, Respect, health and wellbeing, Independence, choices, identity and rights. 1.2 Explain why it is important to work in a way that embeds person-centred values It is important because it will help the individual make decision about their care they are receiving and it will also promote independence. You should also include the resident’s family in planning and maintaining so they are involve with care and satisfied what the individual is receiving. 2.1 Describe how to find out the history, preferences, wishes and needs of an individual When starting the job you should read everyone individual’s cases to make sure you know the history. When you are allocated a person to work with you should read the file in details. When having a key work session and doing the support plan you should ask about the individual preferences and wishes. When doing the risk management you are able to understand the needs of the individual because of the history. This will help the individual because they will able to make decision about what they prefer in there care. You can get this information from the other professional involve with that individual care or previous support worker. You can also ask the family and the GP. 2.2 Describe how to take into account the history, preferences, wishes and needs of an individual when planning care and support Stating facts and quoting words for a word a customer prefers by avoiding to put in opinion. For example customers no longer wishes to, would like to. Using pictures would be useful to identify the specific information as it would make it more descriptive.
