Chapter 13 Assessment of the patient is a key element of nursing care. Signs and Symptoms: In a nursing assessment, signs are objective data as perceived by the examiner. What the nurse sees, hears, measures, and feels are considered OBJECTIVE DATA. Exudate= Drainage refers to the passice or action removal of fluids from a body cavity, wound, or other source of discharge by one or more methods Ex. Closed urinary drainage system or an open drainage system, such as a Jackson-Pratt drain. Symptoms= are subjective indications of illness that the pt perceives - These subjective data are what the patient describes related to pain, nausea and others. Pruritus=itching Disease and Diagnosis: Disease, a pathologic condition of the body, is any disturbance of a structure or function of the body. Origins of Disease: Diseases – Hereditary and sometimes congenital( for some diseases it is unknown ) First two months of pregnancy / drugs alcohol malnutrition and radiation. - Absence of limbs ( structural ) and blindness ( functional ) Neoplastic= disease is described as an abnormal growth of new tissues. The new growth is sometimes benign and sometimes malignant ( Cancerous ) Degenerative disease= Implies degeneration often progressive Infectious disease= invasion of microorganisms in the ody Deficiency disease= results from the lack of a specific nutrient Metabolic disease= dysfunction that results in a loss of metabolic control of homeostasis in the body Ex. Hypothyroidism and acromegaly Traumatic conditions from physical and emotional trauma Environmental diseases are a group of conditions that develop from exposure to a harmful substance in the environment Diabetes mellitus results from the dysfunction of the pancreatic islets Risk factors for disease - Genetic and physiologic - Age - Environment - Lifestyle Frequently noted signs and symptoms of disease: Infection is caused by an invasion of microorganisms, such as bacteria, viruses, fungi or
